Abstract

Diabetes melitus (DM) merupakan terjadi gangguan metabolik yang ditandai dengan hiperglikemia (kenaikan kadar glukosa) di akibatkan kurang hormon insulin, menurunnya efek insulin atau keduanya. DM tipe 2 adalah kelompok DM akibat kurangnya sensitifitas jaringan sasaran (otot, jaringan adiposa dan hepar) berespon terhadap insulin. Penulisan KIAN ini bertujuan untuk melakukan analisis terhadap kasus kelolaan pada klien dengan Diabetes Mellitus tipe 2 yang dirawat di RPDP RSUD Jayapura. Tn. D (43 tahun) merupakan pasien rujukan dari RS Serui dengan keluhan lemas sejak ± 1 minggu yang lalu, kadar Hb 7,3 g/dl, klien tampak pucat, terdapat luka pada jari manis di kaki kiri ± 2 hari yang lalu yang awalnya kutu air, Klien mengatakan tidak mengerti tentang penyakit yang dideritanya, Klien mengatakan sebelum sakit ia tidak memperhatikan asupan makanannya dan Istri klien mengatakan tidak tahu tentang penyakit DM yang diderita suaminya, sehingga tidak kontrol kesehatan ke fasilitas kesehatan. Terapi yang diberikan adalah Diabetes Self Management Education (DSME) dengan hasil evaluasi klien mengatakan paham apa yang telah dijelaskan, klien mengatakan akan patuh terhadap pengobatan yang diberikan oleh dokter maupun perawat dan terdapat perkembangan yang dapat dilihat dari hasil kadar gula darah yang menurun. Kesimpulannya adalah Diabetes Self Management Education (DSME) efektif dalam meningkatkan kualitas hidup pasien. ----------------------------------------------------------------------- Diabetes melitus (DM) is a metabolic disorder characterized by hyperglycemia (increased glucose levels) caused by a lack of the hormone insulin, decreased effects of insulin or both. DM type 2 is the DM group due to the lack of sensitivity of target tissues (muscle, adipose and hepatic tissue) to respond to insulin. The purpose of writing KIAN is to analyze cases of managed with type 2 Diabetes Mellitus who are treated at RPDP Jayapura Hospital. Mr. D (43 years) is a referral patient from Serui Hospital with complaints of weakness since ± 1 week ago, Hb levels 7,3 g/dl, the client looks pale, there is a wound on the ring finger on the left leg ± 2 days ago which initially was water fleas, the client said that he did not understand his illness, the client said before he got sick he did not pay attention to his food intake and the client‟s wife said that she did not know about her husband‟s DM, so he did not go to a health facility. The therapy provided is Diabetes Self Management Education (DSME) with the result of the client‟s evaluation saying that he understands what has been explained, the client says he will comply with the treatment given by doctors and nurses and there is development that can be seen from the result of decreased blood sugar levels. The conclusion is that Diabetes Sel Management Education (DSME) is effective in improving the quality of life of patients.
